HighPoint - Livingston Regional Hospital

The Clinical Admissions Coordinator, in conjunction with the unit’s Program Director coordinates the referral development and pre-admission patient assessment process and provide clinical assessments for potential clients. The RN will communicate with all appropriate parties during the process to assure admissions and discharges are conducted in an effective manner.

Reports to: Director of Oakpoint

Conduct intake referrals, assessments and inquiry summaries and complete admission scales. Works and communicates with the psychiatrist for appropriateness of admissions.

Follow all admission practices and procedures such as securing a legal representative for the patient if the patient is not cognitive.

Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.

Assist social worker in the discharge process. Active participation in treatment teams.

Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.

Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.

Works as an advocate for the physical and emotional well-being of the patient.

Must complete all education assigned and required for the position, including classroom and on-line courses.

Job Requirements

Minimum EducationX Graduate of Program in Discipline - RN X Required

Required SkillsRequires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Certifications:X Basic Life Support (BLS) X Advanced Cardiovascular Life Support (ACLS - within 6 months of hire) __Pediatric Advanced Life Support (PALS) __ Neonatal Resuscitation Provider (NRP) Preferred □ Perioperative Nurse (CNOR) □Other__________________________ □Other_________________________

Licenses: Registered Nurse in State of Tennessee or Compact State

Performs other duties as assigned.

Must work effectively as a team-member.
